What it is
This is your own declaration, on your honour, of the address where you live (or will live) and the capacity in which you occupy it: owner, tenant, sub-tenant, usufructuary or borrower (comodatário — living there with the owner's permission, without paying rent).
It is specifically about the accommodation. It does not vouch for anything else in your file.
When AIMA asks for it
For a renewal, since 18 January 2024 AIMA accepts a sworn declaration naming the legal basis on which you occupy the home in place of a registered lease (Decreto Regulamentar n.º 84/2007, art. 42.º-O). A comodato counts; an atestado from the junta de freguesia does not. The renewal-blockers chapter sets this out with its sources.
It is also asked for in family reunification and first-permit files. If someone else houses you, the document AIMA wants from them is the hosting declaration.

The wording
The document AIMA receives is in Portuguese. This is the text the template produces; the blanks are the fields explained below.

What it says, in English
I, [Your full name], holder of identification document no. [Identification document (number)], declare on my honour that I reside (or will reside) at the accommodation located at:
